Reducing Avoidable Rehospitalizations
Our APRNs work alongside facility teams to identify clinical changes early, strengthen follow-up, and coordinate care transitions to help reduce avoidable rehospitalizations.
Proactive Health Care Management
Proactive Health is a group of nurse practitioners with experience in post-acute care, long-term care, and skilled nursing.
The mission follows patients from hospital discharge through post-acute care and, when possible, a safe return home.
We help patients move from the hospital through post-acute care with one clear goal: a safe, well-supported return home whenever possible. Along the way, our nurse practitioners work closely with facility teams, families, and community physicians.
We want every facility we serve to have practical clinical support, consistent communication, and providers who understand the day-to-day realities of post-acute care.
Our clinical partnerships focus on practical improvements that support residents and facility quality goals.

Through consistent clinical support, communication, and facility-selected quality review, we help leadership teams work toward stronger performance and their CMS Five-Star goals.
These five ideas come from the original Proactive Health model. They set expectations for how our providers work with patients, families, facility staff, and community physicians.
Treat each patient with dignity and keep their needs at the center of the plan.
Follow through for patients, families, and the facility teams caring for them.
Bring sound clinical judgment and post-acute experience to each decision.
Keep families, facility staff, and community physicians informed.
Work with the people already involved in the patient's care and discharge plan.
