Public forms are for business contact and referral callback requests only. Do not submit patient names, dates of birth, diagnoses, records, or other protected health information through this website.

Information We Collect

When you submit a callback request, partnership inquiry, or career application, we collect the business or applicant contact information you provide so our team can respond. Career applications also accept a resume file, which is emailed to the hiring team and is not stored on this website. Public forms are not intended to receive patient information.

Protected Health Information

Do not submit patient names, dates of birth, diagnoses, clinical notes, records, or other protected health information (PHI) through this website. The referral form requests a callback only. Our team will coordinate the appropriate referral process after making contact.

Form Delivery

The website does not provide a patient portal or public application dashboard. When email delivery is configured, form submissions and attached resume files are sent to the designated business email address. Submitters also receive a confirmation at the email address they provided. Resume files are not stored on the website. The public forms are not approved for PHI transmission.
